If you are an SOTV member or your non-profit organization would like to receive more information about holding an event at SOTV, complete the following information below. Organizations must be a non-profit 501c3 to qualify for a Facility Use Agreement. Fees are applicable.
Facility use requests must be received no later than 2 weeks before a personal event, 4 weeks before a community or organization event, and not more than 6 months prior to your requested date of your event.
Submission of this form does not guarantee your space. An SOTV representative will respond to completed requests submitted adequately prior to the requested event date.
